Please read carefully before filling out entry form.

1.      Have students fill out a "HISTORY DAY L.A. STUDENT ENTRY FORM" (white) for each project entered in the contest.  These should be TYPED OR PRINTED LEGIBLY IN BLACK INK.  All information requested should be provided.

         PLEASE CHECK FOR LEGIBILITY BEFORE SENDING.

         You may make as many copies as needed of the entry form.  NO SCHOOL OR TEACHER MAY SUBMIT MORE THAN THE FOLLOWING NUMBER OF ENTRIES.

               Elementary Division (Grades 4-5):     Each school may submit 2 individual poster entries and 2 group poster entries per number of fourth and/or fifth grade classrooms.

               Junior Division (Grades 6-8):              Each school may submit 5 entries per category

               Senior Division (Grades 9-12):            Each teacher may submit 5 entries per category

         Entry forms for group poster, exhibit, performance, documentary, and web site should include the names of all the members of the group.  Only 1 form needs to be submitted for the group.  A group leader should be selected.  His or her name should go on line #1 where it says "Student's Name."  This student will be responsible for signing the form and obtaining his or her parent's signature. 

2.      Teacher should fill out the "TEACHER ENTRY FORM" completely.  Include the total number of students entered.  Each student in a group poster, exhibit, performance, documentary, or web site is to be counted separately.

         ATTACH THE FOLLOWING TO THE "TEACHER ENTRY FORM":

               1.      "HISTORY DAY L.A. STUDENT ENTRY FORMS" for every poster, paper, exhibit, performance, documentary, and web site entered.

               2.      A school check or money order covering the fees ($40.00 per student) for every student entered.  Please collect money from students and send a school check or money order to cover all entrants.  Personal checks will NOT be accepted.  SCHOOL CHECKS or money orders should be made out to:  Los Angeles County Office of Education.

3.      DEADLINE:  All forms, fees, historical papers and website materials must be received by 4:00 p.m. on Friday, February 4, 2011.
